Submit Event Request Form

  • Events will be posted within 72 business day hours (minus weekends) of submission of completed events only.
  • Events with partial information will not be entered into the system and returned for missing or incomplete information. Anything listed as optional is not a required field.
  • CSCMP reserves the right to make any adjustment to ensure CSCMP standards are met.
  • All webinars must include the link to the event to be considered complete.
  • Changing the date of an event will require that event to be closed and a new event will be created.
  • Any payments received against the closed event will need to be refunded.
  • Events must have complete information 6-8 weeks from the event start date.

If your event is returned for missing information, it will be returned to Pending status. You will need to enter the missing information and returned to Submitted to CSCMP status for CSCMP to add to system within 2 business day hours (minus weekends).

Submit Event Request Form

Access the Roundtable Board Main Dashboard. On the bottom right locate the Reports & Forms section and select 'Submit Event Request Form' link.
To fill out the form (add a new event request), click the Plus Icon.


Event Name

There is a max of 60-character limit including spaces.

Description Summary The event description is essential for promotion and allows attendees and supporters to prioritize your event as a business objective. Write a high-level overview that clearly defines your event vision, key takeaways and the impact it will have by attending.
Please note:
  • Event descriptions are required for approval
  • Descriptions will display in 3 places: the Roundtable Events listing page, the individual roundtable’s public page, and the specific event details page
  • There is a limit of 700 characters, including spaces. You may enter more characters; however, your customers will not see the entire description.
Event Details (Optional) You may upload a Word document file that will be used to add HTML formatted content for the main event display. Keep in mind the details you placed in the Summary will show above this Event Details area. Limited to an additional 1500 characters with spaces.
You are allowed to add the following formatting only:
  • Bold
  • Italicized
  • Bullet listings
Event Date(s) and Time(s) Enter the date of your event and the start and end times your event will be hosted.
Event Location Enter the name of the venue, full address a room name/location. For virtual events: please only enter the state your roundtable resides.
Event Capacity Enter the headcount your event can hold. Registrations will not be accepted beyond this number.
Forecasted Number of Attendees Enter the number of expected to attend, including volunteers, sponsors, complimentary registrations, and unique registrations. Consider referring to your roundtable’s attendance records from the last 2-3 years as a basis for the type of event you are hosting.
Main Volunteer Contact Enter the contact information (name, email, phone) for the lead volunteer available and able to answer questions about the event either from staff or customers. The phone is for staff use only and will not be displayed.
Pricing Enter the price to attend your event for each member category. Pricing must reflect the quality of event being hosted, cover costs and provide a cushion to cover your next event and any other annual objectives set by your roundtable. Never consider sponsor/support dollars into your event rate.

Virtual Events: Charging members for virtual events is at the roundtable's discretion; however, non-members must be charged a minimum of $25.
In-Person Events: All member categories and guests must be charged.
  • Must include a Base Price, which is used as the member rate.
  • NonMembers must be a 50% increase to the Base Price rate (can round to the nearest whole dollar).
  • Young Professionals and Students do not have a pricing requirement other than the Student must be the lowest price.

Event Type Select the type of event you are hosting from the drop-down list provided.
Sponsorship Are you seeking sponsorship for this event?
URL to Join Virtual Event Enter the URL/event link attendees will use to access the event. Note: The event URL is part of the receipt process for your attendees; event requests will be declined without the URL.
Status Pending - You are not ready for CSCMP to create the event just yet, and still working.
Submitted to CSCMP - Your event is ready to be created. Any changes or updates moving forward will need to be completed via the Change Request Form.
Event Creation in Progress - CSCMP has seen your request and is processing your request. Note: There is a 72-hour turnaround on event requests. Please plan accordingly.
Event Created - Your event has been created and registration is now open. This event will be listed in all designated areas.
Declined - There is an issue with your request. You will be contacted directly.
iMIS Event Code A code will generate once your event has been created. This code is important for future reporting and for your reference.
